12-Month Continuation Program

Enrollment Agreement

This Continuation Program Agreement ("Agreement") is entered into by Anzara Health PLLC ("Company") and the undersigned individual ("Client"). This Agreement governs Client’s participation in the Anzara Health 12-Month Continuation Program following successful completion of the Anzara Health 3-Month Coaching Program.

1. Eligibility

The Continuation Program is available exclusively to clients who have completed the Anzara Health 3-Month Coaching Program. Enrollment in this Program confirms that the Client has previously completed the 3-Month Coaching Program and had a personalized meal plan and workout plan developed during that program.

2. Program Overview

The 12-Month Continuation Program is a prepaid, fixed-duration app access plan. The Program begins on the date this Agreement is accepted and concludes at the end of the 12-month program period. The Program provides self-guided access to the personalized plans created during the Client’s 3-Month Coaching Program and does not include any coaching sessions or direct communication with the Anzara Health team.

3. Services Included

•        Access to the Anzara Health app for the full 12-month program term

•        The personalized meal plan developed during the Client’s 3-Month Coaching Program

•        The personalized workout plan developed during the Client’s 3-Month Coaching Program

•        Progress tracking tools within the Anzara Health app

4. Services Not Included

The Continuation Program is an app-access plan only. The following services are not included and are not available under this Agreement:

•        Registered Dietitian sessions or consultations of any kind

•        Physical Trainer sessions or consultations of any kind

•        Messaging check-ins or coaching team communication

•        Meal plan or workout plan revisions or updates

•        Physician or endocrinologist consultations

•        Medical evaluations or diagnostic services

•        Medication prescribing or management

•        Prescription refills

•        Diagnosis or treatment of medical conditions

•        Emergency medical services

If the Client wishes to resume RD or PT coaching services, a new Coaching Program Agreement would be required.

5. Nature of Services

The Continuation Program provides access to previously developed coaching tools and content for self-guided reference only. It does not constitute ongoing coaching, medical advice, or professional health services. The Client is solely responsible for how they implement any plans or content accessed through the app.

6. App Access & Data Termination

Client’s access to the Anzara Health app and all associated program data — including meal plans, workout plans, and progress tracking information — will remain active through the end of the 12-month program term. Upon expiration, app access and all associated data will be discontinued and will no longer be accessible to the Client. Data is not transferred, exported, or retained beyond the program term.

7. Confidentiality

All confidentiality terms from the Client’s original 3-Month Coaching Program Agreement remain in effect under this Agreement, including but not limited to:

•        Program materials, app content, meal plans, and workout plans may not be shared with friends, family, or acquaintances.

•        Progress photos will not be released without explicit written consent from the Client.

•        Messages and results (excluding progress photos) may be shared anonymously for marketing purposes.

8. No Guarantee of Results

Anzara Health PLLC does not warrant or guarantee any specific level of performance or results during the Continuation Program. The effectiveness of the Program depends entirely on the Client’s own implementation of the plans and tools provided.

9. Payment & Cancellation Policy

The 12-Month Continuation Program is offered at a one-time prepaid fee. The following terms apply:

•        Payment: A single upfront payment is required to enroll.

•        No Auto-Renewal: This Program does not automatically renew. No additional charges will occur after the one-time payment.

•        Cancellation: The Client may cancel at any time by notifying Anzara Health PLLC in writing. Cancellation will discontinue future access but will not result in a refund of fees already paid.

•        No Refunds: Because access to the app and its digital content is granted immediately upon purchase, refunds are not provided for any payments already made.

•        Service & App Expiration: All app access and associated program data expire at the end of the 12-month program term regardless of usage. Upon expiration, access to the Anzara Health app and all associated data will be discontinued.

10. Governing Law & Entire Agreement

This Agreement shall be governed by and construed in accordance with the laws of the United States. Exclusive jurisdiction and venue for any action arising under this Agreement shall be in the applicable national court, and both parties consent to such jurisdiction. The prevailing party in any enforcement action shall be entitled to recover reasonable attorneys’ fees and costs.

This Agreement, together with the Client’s original 3-Month Coaching Program Agreement, constitutes the complete and exclusive agreement of the parties with respect to the Continuation Program. No modification shall be effective unless in writing and signed by both parties.

Waiver and Release of Liability

Client releases Anzara Health PLLC from any liability for injury or loss resulting directly or indirectly from use of the Continuation Program. Client acknowledges that use of the app, meal plans, and workout plans is entirely at his or her own risk, and that Client bears complete responsibility for the consequences of any action or failure to act in connection with the Program.

I understand that this Program does not replace the care of my physician or healthcare professional, and I agree not to disregard or delay seeking professional medical advice, or stop taking any medications, without first consulting my physician or healthcare professional. I agree to disclose to Anzara Health PLLC any known or suspected food allergies or sensitivities, physical limitations, or health or mental conditions that may affect my participation in the Program.

Client acknowledges that participation in physical exercise and nutrition programming involves inherent risks. Client voluntarily assumes all such risks of injury or loss and releases Anzara Health PLLC from any liability arising directly or indirectly from use of the Continuation Program, regardless of whether such injury was related to a disclosed or undisclosed condition.
