Align - 3-Month Coaching Program
Enrollment Agreement
This Program Agreement ("Agreement") is entered into by Anzara Health PLLC ("Company") and the undersigned individual ("Client") and governs participation in the Anzara Health Align 3-Month Coaching Program.
1. Program Overview
The Anzara Health Align 3-Month Coaching Program ("Program") is a prepaid, fixed-duration coaching service combining Registered Dietitian (RD) and Personal Trainer (PT) coaching services delivered through the Anzara Health App. The Program begins on the date the Client accepts this Agreement and concludes at the end of the 3-month program period.
The Program is a coaching service only. It does NOT include physician or endocrinologist consultations, medical evaluations, medication prescribing or management, prescription refills, diagnosis or treatment of any medical condition, or emergency medical services.
2. Program Services
What Is Included
All sessions are conducted virtually through the Anzara Health App
• One (1) Registered Dietitian Assessment Session (45 minutes, virtual)
• Two (2) Registered Dietitian Follow-Up Sessions (30 minutes each, virtual)
• One (1) Personal Trainer Assessment Session (45 minutes, virtual)
• Two (2) Personal Trainer Follow-Up Sessions (30 minutes each, virtual)
• Weekly messaging check-ins through the Anzara platform. Response times are typically within 24–48 business hours. Messages received on weekends or holidays will be responded to on the next available business day.
• Personalized meal plan recommendations and revisions
• Personalized workout recommendations and revisions
• Progress tracking and accountability support
• Access to the Anzara Health app, where all program materials, session notes, meal plans, workout recommendations, and progress data are stored
What Is NOT Included
• Physician or endocrinologist consultations
• Medical evaluations or diagnostic services
• Medication prescribing or management
• Prescription refills
• Diagnosis or treatment of medical conditions
• Emergency medical services
All sessions, services, app access, and program data expire at the end of the Align 3-month program period. Unused sessions do not carry over. Upon expiration, the Client’s access to the Anzara Health app will be discontinued and all associated data stored within the platform will no longer be accessible to the Client.
3. Client Requirements
By enrolling in the Program, Client agrees to:
• Provide any required health and personal information necessary for effective service delivery.
• Complete program exercises, attend scheduled sessions, and adhere to the agreed-upon plan.
• Perform any "Action Items" as assigned through the Anzara Health app.
•Disclose in advance any known or suspected food allergies, physical limitations, or health or mental conditions that may affect participation in the Program.
• Notify Anzara Health PLLC immediately if a medical problem is suspected during the Program.
4. Nature of Services
Services include educational content, tools, and general guidance related to nutrition, physical activity, and behavior change in support of the Client’s health goals. Services may also include prompts, questions for self-reflection, suggested action steps, and general recommendations designed to support the Client’s decision-making and progress.
The Client agrees that achieving results requires a proactive approach and that it is the Client’s responsibility to implement any actions or recommendations provided through the Services.
5. Confidentiality
During this Agreement, either party (the “Disclosing Party”) may share private or sensitive information with the other (the “Receiving Party”). The Receiving Party agrees to keep such information confidential and not share it, except as required by law. This obligation does not apply to information that is already public, was legally received from a third party, was already known, or was independently developed without use of the shared information.
• Progress photos are for the Coach and Client to review together. No progress photos will be released without explicit written consent from the Client.
• Program materials, content, and platform access may not be shared with friends, family, or acquaintances.
• The Anzara Health PLLC team will report imminent self-harm, abuse, or harm to others as required by applicable law.
• Messages and results (excluding progress photos) may be shared anonymously for marketing purposes.
6. No Guarantee of Results
Anzara Health PLLC does not warrant or guarantee any specific level of performance or results. Examples of results obtained by other clients may be used for demonstrative marketing purposes only and should not be construed as a promise of any particular outcome.
7. Payment & Cancellation Policy
The Program is offered at a one-time prepaid fee covering the full 3-month program period. The following terms apply:
• Payment: A single, upfront payment is required to enroll in the Program.
• No Auto-Renewal: This Program does not automatically renew. No additional charges will occur after the one-time payment.
• Cancellation: The Client may cancel at any time by notifying Anzara Health PLLC in writing. Cancellation will stop access to future services but will not result in a refund of the one-time fee already paid.
• No Refunds: Because access to the Program, its content, and digital services is granted immediately upon purchase, refunds are not provided for any payments already made.
• Service & App Expiration: All services, sessions, app access, and program data expire at the end of the 3-month program period, regardless of whether all sessions were used. Upon expiration, access to the Anzara Health app and all associated data will be discontinued.
8. Entire Agreement
This Agreement is the final, complete, and exclusive agreement of the parties with respect to the Program. No modification or amendment shall be effective unless in writing and signed by both parties.
9. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the United States. Exclusive jurisdiction and venue for any action arising under this Agreement shall be in the applicable national court, and both parties consent to such jurisdiction. In any action to enforce this Agreement, the prevailing party shall be entitled to recover reasonable attorneys’ fees and costs.

Waiver and Release of Liability
Client releases Anzara Health PLLC from any liability for injury or loss resulting directly or indirectly from participation in the Program. Client acknowledges that participation in the Program is entirely at his or her own risk, and that Client bears complete responsibility for the consequences of any action or failure to act, whether or not suggested by the Company in the course of providing Services.
I understand that this Program does not replace the care of my physician or healthcare professional, and I agree not to disregard or delay seeking professional medical advice, or stop taking any medications, without first consulting my physician or healthcare professional. I agree to disclose to Anzara Health PLLC in advance any known or suspected food allergies or sensitivities, physical limitations that may impact my breathing or movement, or any other health or mental condition that may affect or be affected by participation in the Program.
Client acknowledges that participation in physical exercise and nutrition programming involves inherent risks. Client voluntarily assumes all such risks of injury or loss and releases Anzara Health PLLC from any liability arising directly or indirectly from participation in the Program, regardless of whether such injury was related to a disclosed or undisclosed condition.
